How to turn a PDF into a fillable form for free with Sejda

Go to sejda.com and select the Edit menu. You do not need an account. The free tier allows three tasks per hour.

Upload the PDF you want to make fillable. It opens in a preview, and you can zoom out to see the whole page.

Open the Forms menu and choose Text. Click where the field should go, then drag its edge to set the width. Repeat for each line such as name, email, phone, company and monthly income.

For a large notes area, use the Text multi-line option instead so people can type several lines.

From Forms, choose Drop-down list and place it. Select Field properties to set the field name (Profession in the video) and type the options, for example Self-employed, Working in a company and Not working.

Tick Allow multiple selections only if people should be able to pick more than one option. Click outside the field to finish.

From Forms, choose Radio button and place one next to each choice, such as the three gender options. Resize each button to fit. People can pick only one radio button in a group.

From Forms, choose Checkbox and place one next to each option, such as email, phone and text for the preferred contact method. Make them smaller if needed. People can tick as many checkboxes as they like.

Select Apply changes, then Download to save the fillable PDF.

Open the downloaded PDF in Foxit PDF Reader. Type in the text fields, pick from the drop-down, choose a radio button and tick the checkboxes.

To keep the blank form, use File and then Save As, and save the filled copy under a new name. Otherwise you overwrite the original.

Open the blank form in Adobe Acrobat Reader too. The fields work the same way, so recipients can use whichever PDF reader they have.
